    Is anyone aware of any government agencies that issue country of origin certificates? My management has asked for these and I do not have any experience with a certificate like this besides CFG or FSC, but I also don't consider these "country of origin" certificates. Any advice would be greatly appreciated.

    Depends on the country, and also may depend on why you are trying to get this certificate of country origin.  You are correct a CFG and FSC are not a country of origin certificate, these are certificates showing the device has gone through a regulatory process - but they are also not any form of approval or acceptance either.  In most cases you would get a certificate of country origin through (in the United States) you local Chamber of Commerce and in other countries would be like Chamber of Industry or equivalent.

    These are completed by the company who manufactures the product for customs duty purposes. You have to declare from where the raw materials originate for the finished product and apply the appropriate HTS Code for the country to which the product is being exported.  The website is a good resource for most of the information you require.  This, in conjunction with the Customs website for the specific country, will assist in completing the document.  Some countries have required formats for the form such as the NAFTA agreement for Mexico and Canada, whereas other countries such as Australia have no specific format so you can easily find a generic CoO template on the internet.  I have attached a sample of one that can be used for several countries.

    We issue these as blanket at the end of each year for the following year so the customer isn't requesting one with every order they place.  However, we do include a copy with each set of shipment documents in order to aid clearance in the foreign country.
