Hello-
I am seeking to renew our Japanese FMA certificate. My understanding from the PMDA website, is I need to submit FRM 20.- Application for Renewal of Accreditation. Where is can this FRM be found? I've searched the PMDA website, and I am having no luck. Secondly, along with the form I need to submit our previous certification. Based on the snippet from the website, no other documents need to accompany the request-is that correct?
Lastly, where do I send the completed application and accompanying documents? Is it simply faxed to the division below?
Any feedback / guidance would be greatly appreciated!
