    Dear all,

Please I need your support on below point.

We want to create an imaging order form with our logo and it will be used as communication tool with the physician for imaging request.
But I wondering if it acceptable. I saw some company which done it and I want to be sure if  it's allowed to use this king of document for advertising.

The reason why I'm asking this question is that, I've read in Quebec Regulation respecting standards relating to prescriptions made by a physician
    Medical law that :
The order must not contain a name or logo of products, services or suppliers of products or services.
A physician who uses a technological tool to prescribe a prescription must ensure that the application, including decision support tools, does not disseminate or encourage the dissemination of any form of promotion of specific products, services or providers of products or services.

According  to my understanding this sentence applies to pharmaceutical  prescription but may be I'm wrong.

    Please could you advise.

Thank you in  advance for your help.
