    Dear Raps Community,

    I am writing regarding issue in creating cover sheet for the Medical device user fee payment, when i proceed after selecting that establishment user fee is paid, then application is not going further, but directing it to following error.

    I would really appreciate if someone could guide here, i did not have issue in January this year for the 510(k) user fee payment.

    Hello Rabel,

    Personally I have not seen that error before, but if you reach out to the FDA help desk, they are usually quite responsive and help with fixing the error.
